Polar residual functor R = -div_polar F + S (fill_ghosts then assemble_rhs_polar).

NAMED FUNCTOR (counterpart of cartesian detail::BlockRhsEval): this is what take_step receives, triggering the instantiation of assemble_rhs_polar<Limiter, Flux> and its device kernels. wall_radial: solid radial wall (no-penetration) -> mass conserved to machine precision (see assemble_rhs_polar).
